The Fabio Sirna site has a lovely set of PDF files that you can print and then cut and fold into a set of envelope-corner style of bookmarks. One set of bookmarks has a saying, another has a set of lines for making notes, and the third has a set of squares, like graph-paper - I guess for drawing a quick graph to chart your reading (okay, maybe too obsessive).
Another page on that site has a photograph based bookmark that you can print on photo paper and assemble.
All the instructions for these bookmarks are in English, but the site itself appears to be Italian.
